Fill out this form. When it is complete, print it out and hand it in with the $40 membership fee. Filling out the form does not make you a member - you become a member once the form and fee are both turned in to a board member or the Circle K office.
The information provided for the directory section (the fields with a *) will be available to MEMBERS ONLY unless you specify that it should be kept private. Your information will never be available to anyone who is not a paid member.
If a printer is not available, a copy of this form will be sent to you via email so you can print it at a later time.
